[Accessibility-testing] Survey First Draft, A Night Beneath the Opera

I have written up my game-specific questions below. Sorry about missing my 
deadline of last night!

I think that by combining these questions with Zack's set, we have a good 
survey for Opera testers.

Game progress questions:

* Overall

Were you able to reach all the locations? (The Kitchen, the Washroom, the 
Storage Room, the Smoking Room, the Rear Corridor, the Basement, the Dance 
Floor.)

How many of the the Items of Darkness did you acquire? Which ones would 
have been entirely unattainable if you had not had the walkthrough?

(If you completed the game:) Did you reach the ending marked "You have 
tidied", the ending marked "You have collected", or both?

* The Washroom

How many of the signs were you able to read clearly? (We won't ask how 
many you understood, since they are in several languages!)

Were the emoji on the emoji sign readable?

Were the boldface and italic words on the font sign distinguishable?

Each time you pulled the lever, were you able to read your total score? 
(Distinct from the "Your score has just gone up by one point" message.)

* Storage Room

Were you able to crumpled paper, and understand it as a map with a path 
drawn across it?

Were you able to open the holiday card, and understand the sequence of 
clicks that it plays? (Either auditorially or through alt text.)

* Smoking Room

Were you able to read the paragraph of text contained in the book?

Were you able to examine the three photographs? Were you able to 
understand the console photograph in enough detail to learn the six 
numbers shown on it? (Either visually or through alt text.)

(If you read the description of the last slide in the slide projector:) 
Were you able to follow the instructions and locate the Left Glove of 
Darkness?

* Rear Corridor

Were you able to turn on the slide projector and read the descriptions of 
the six slides?

Were you able to clean the plaque and read the verse printed on it?

(If you read the verse:) Were you able to follow the instructions and 
acquire the Earring of Darkness?

* Basement

(If you understood the six numbers in the console photograph:) Did you 
understand what to do with the buttons to open the panel and acquire the 
Cravat of Darkness?

(If you understood the click sequence in the holiday card:) Were you able 
to push the buttons in sequence to turn on the white spotlight?

* Dance Floor

Were you able to move around the twelve locations of the dance floor 
without getting disoriented?

(If you understood the map path on the crumpled paper:) Were you able to 
follow the path and acquire the Right Sandal of Darkness?

(If you turned on the white spotlight:) Were you able to follow the 
spotlight to its source and acquire the Leg-Warmer of Darkness?
